Floyd County Christmas for Children Application
The purpose of Christmas for Children (CFC) is to provide supplemental gifts during the holidays for children (age newborn to high school senior and unmarried) of families who are of low income or experiencing financial problems and live in Floyd County. The primary focus of CFC is on the needs of low income children. CFC intends to treat everyone as fairly as possible and to do our best to meet reasonable requests listed on applications. CFC does not accept requests for bikes, food, or high-dollar electronics, there are other organizations to help with these items. CFC is not limited to families receiving public assistance. During the application screening process additional information may be required in order to process the application. After submitting this application, if there is a change of address or contact information, please notify the CFC committee immediately by calling 540-745-3370. Applications are kept confidential. Information provided is only available for processing.

I understand that submission of this application does not ensure I will receive Christmas for Children assistance. I understand that my application will be reviewed and evaluated, and that I will receive timely notification, in person or through my preferred method of communication, of its acceptance or rejection.
I authorize Christmas for Children and its volunteers to obtain and/or release information relating to this application among agencies providing services for Floyd County children similar to those services provided by Christmas for Children. Such agencies include, but are not limited to, Floyd County Department of Social Services, NRV Floyd Community Action, United Way, churches, schools and other organizations. I agree that CFC may contact me during this application process for additional information.
By signing below, I affirm that the information provided in the application process is true and correct. I understand that if I misrepresent any information on this application, then I will be permanently banned from the CFC program.

Floyd County Christmas for Children Application
This program focuses on basic needs. Since this is a supplemental program, CFC intends to assist but cannot provide what may be called a *total Christmas package*. As funding allows CFC will provide for each child an outfit of clothing (to include a shirt, pants, underwear, socks and shoes) and at least 1 *Wish List* gift.
The parent/guardian is asked to provide CFC with a “Wish List” of up to 4 choices of toy or fun gift for each child. (This list is to be completed within the application.) Please be very specific. CFC cannot guarantee any wish will be granted but the volunteers will make every effort to fulfill at least 1 gift choice, as long as there are resources to do so.
- Requested “Wish List” items must be of reasonable value. Please be aware that CFC may not be able to provide expensive name brand items.
- Each child's clothing sizes MUST be provided. Be as detailed as possible when giving sizes.
- To allow for growth, give sizes that will be useful to the child 3 to 6 months from the application date.
- Provide as much information as possible for each “Wish List” item to help the volunteer shopper choose the best gift.
- If there are symbols or logos the child would like (and that the parent/guardian approves) indicate on the “Wish List.”
- If there are symbols or logos the parent/guardian DOES NOT want on gifts purchased, indicate on the “Wish List.
- Christmas for Children does not provide bicycles.
